HOW TO RECEIVE HOSPICE CARE?
A physician orders hospice at the request of a patient, family member, or medical representative such as a discharge planner, case manager, or social worker. Call our office for more information. Team members will assist you with the process and are available for information visits in the patient’s place of residence, hospital or physician’s office.
